How to Refer a Patient

Referring a patient to the St.Vincent Bariatric Center of Excellence for a consultation is easy, and can be done by any of the following methods:

1. Complete the Surgical Patient Referral form and fax to us at 317-582-7068.
2. Call us at 317- 582-7088—or toll free at 1-877-831-1582.
3. E-mail us at bariatrics@stvincent.org.
